Care Connect+ Partners deliver invaluable services to our shared clients every day.  The Partner Appreciation Conference will give us an opportunity to connect, share success stories and derive feedback from our network partners while recognizing the incredible work accomplished in our community.  

Agency Executives and Staff are invited to an afternoon conference on Thursday, April 7th, 2022 from 2-6pm to learn about Care Connect+ future plans, provide feedback on how we can improve to meet partner agency needs, and network with other agencies.  Please RSVP By April 1, 2022 through this registration form.
