Rich history mixed with innovative technology is an integral part of Flagler Hospital in historic St. Augustine, Florida. Since its founding in 1889, Flagler Hospital is a private, not-for-profit facility that has grown into a 335 bed, acute care center.
Flagler Hospital has consistently ranked among the top 5% in the nation for clinical excellence and patient safety for nearly a decade and was Northeast Florida’s first hospital to receive the ANCC Magnet hospital award. Most recently, the hospital earned five stars for heart attack treatment, the Gold Seal of Approval™ from The Joint Commission for Primary Stroke Care Centers, Center of Excellence Designation for its Bariatric Surgery Center, and national accreditation for total knee and total hip replacement surgery.
Flagler Hospital established Care Connect to support its vision to become a national model for community-based care.
St. Johns County Health and Human Services
The Board of County Commissioners funds the St. Johns County Health and Human Services Department, which offers social support and assistance to needy members of the community by administering the County’s assistance programs.
Community Hospice and Palliative Care
Established in 1979, Community Hospice & Palliative Care is an innovative national leader in palliative and hospice care, providing programs and services to meet the diverse needs of the communities it serves. During its 39-year history, the community-focused nonprofit organization has served nearly 200,000 adults and children in Baker, Clay, Duval, Nassau and St. Johns counties and recently expanded its service area to include 11 counties in north-central Florida. Through the support of 950 employees and nearly 1,000 volunteers, the organization helps approximately 1,600 patients per day live better with advanced illness – at home, in long-term care and assisted living communities, in hospitals and at its eight inpatient care centers. The organization is committed to improving the quality of life for patients and their families and being the compassionate guide for end-of-life care in our communities. Honoring Choices Florida
Advance care planning (ACP) is a process of conversation and documentation, providing you with the opportunity to create well-informed plans so that your health care providers and family know your wishes in the event you could not speak for yourself.
Fifty percent or more individuals are not able to make their own decisions when critical health decisions need to be made. ACP is for every capable adult, regardless of age and health status. Honoring Choices Florida is a free community-wide resource to help you discuss, decide and document your wishes for care in the event of future health challenges. Staff is available to help you make informed and thoughtful decisions about the care you want when it matters most.

Sertoma
Sertoma is a non-profit organization dedicated to the betterment of those less fortunate and specifically those with medical or financial needs provided that all monies spent stay in St. Johns County Florida. Through many fundraising efforts and the generosity of our local community, Sertoma’s financial outreach serves locals who fall through social safety nets, identifies local civic needs and Sertoma works with other local charities to fulfill their goals. Members of Sertoma believe in the compassion and sacrifice of the human spirit and the devotion to good causes through financial discipline and sheer will power and faith in one another.

Alpha-Omega Miracle Home
Alpha-Omega Miracle Home provides resources for new beginnings for single mothers, their children and senior women. Their programs and services provide: housing, counseling, spiritual guidance, education, employment assistance, diaper bank, thrift store vouchers and adoption assistance.
Mission: To demonstrate God’s love through transitional housing, education, vocational assistance, and counseling to those who are most vulnerable single and expectant mothers, children and the elderly.
Catholic Charities - St. Augustine
Catholic Charities of St. Augustine provides services to families, individuals, seniors on fixed incomes, persons with disabilities or chronic illness, victims of violence and emergency situations, employed individuals. Catholic Charities offers programs and services for food assistance, mobile food pantry, weekend hunger backpack, and homeless prevention. Along with adoption and pregnancy counseling.
Mission: Catholic Charities puts faith into action to serve the most vulnerable in our community, advocating for justice, human dignity and quality of life, while reflecting the compassion of God in Christ.
ESHC of St. Johns County
Emergency Services & Homeless Coalition of St. Johns County focuses on: breaking the cycle of poverty and homelessness with children, building skills to help give homeless families a positive future, providing safe housing for homeless children and their parents.
Mission: The mission of ESHC of St. Johns County Inc. is to truly make a difference in the lives of homeless children and their families by providing the opportunity for them to remain intact, stabilize their lives, save towards affordable permanent housing and break the cycle of homelessness.
Wildflower Healthcare
Wildflower Healthcare is the leading non-profit provider of free medical and dental services to low income, uninsured adults in St. Johns County. They ensure access to quality, compassionate healthcare that improves the lives of people they serve.
Home Again St. Johns
Founded in 2011, Home Again St. Johns is a non-profit organization coordinating a multi-agency system to positively effect and alleviate homelessness in St. Johns County through the active engagement of those residents experiencing homelessness: individuals, families, veterans, community as a whole, working together in supporting their return to self-sufficiency.
The Salvation Army
The Salvation Army began serving the people of northeast Florida in 1891. Since that time, they have been providing food, clothing, shelter and countless services to assist those in need.
Mission: The Salvation Army is an international movement, an evangelical part of the universal Christian Church. Our mission is to preach the Gospel of Jesus Christ and to meet human needs in His name without discrimination.
EPIC Behavioral Healthcare
EPIC Behavioral Healthcare serves as a source to the community on the focus of quality substance abuse and mental health services. EPIC serves children, adolescents, adults and seniors. They provide a range of services including: counseling, prevention and recovery.
Mission: To strengthen our community by assisting individuals and families impacted by drugs and alcohol, mental health disorders and related problem behaviors through a comprehensive system of Education, Prevention, Intervention and Counseling.
St. Francis House
St. Francis House provides services and programs to the homeless population. Services and programs are: housing crisis, street outreach, Port in the Storm, advocacy and referrals.
Mission: The St. Francis House Mission is to engage the homeless in achieving stability, self-sufficiency, and a viable plan for their future by providing a safe, stable environment, access to essential services, supportive connections, and to advocate for changes that address the needs of the homeless population.
Changing Homelessness
Changing Homelessness is an organization that advocates and educates to end homelessness. Changing Homelessness is a part of The Department of Housing and Urban Development (HUD) supportive services for veteran families (SSVF). Changing Homelessness was formed in 1974 of concerned and engaged social services agencies and religious leaders. They came together with the vision to end homelessness.
Mission: The mission of Changing Homelessness guides the community effort and builds capacity to prevent and end homelessness.
St. Johns County Council on Aging
The St. Johns County Council of Aging is a council determined to help. The Council of Aging (COA) provides a wide array of services. Some services include: memory services, Meals on Wheels, care connection and more. The COA also has an adult day care center to help participants enjoy exercises, socialization, activities and skilled care.
Mission: To lead and advocate for the dignity, independence, health and community involvement of older St. Johns County residents.
